Mulligans are enemies with tear-filled eyes and a bloated head that appear in Chapter 1. Mulligans are afraid of the player and will actively try to avoid them. Their eyes widen and their speed increases when evading a nearby player. In their panicked state they may stumble over spikes or into fire.
Mulligans may self-detonate. Upon death they can spawn several Black Flies, Attack Flies and occasionally a Pooter. Mulligans cannot deal contact damage to the player unless
they are champions.
In later chapters, deformed variants of Mulligans will appear.

  Chance to replace Mulligans
|
Mulligoons display a depressed appearance, but unlike Mulligans, they do not have tears in their eyes. They will audibly wail and attempt to evade the player. Their skin is a darker shade and one eye appears to be missing. They also don't generally hurt the player on touch and may self-detonate. They release 4 projectiles in an X-pattern and drop a troll bomb on death. If they self-detonate, they release 8 projectiles rather than 4.

  Rare chance to replace Mulligans or Mulligoons
|
Mullibooms have a bomb wedged into their head. Instead of avoiding the player like all other Mulligan-types, they will charge at the player and explode on contact, dealing half a heart of damage. They also explode immediately if they are killed, similar to Boom Flies. Make sure to keep a safe distance when delivering the final shot, and consider killing them next to a wall that might be adjacent to the Secret Room.
Hives are enemies with a large rotting head and no eyes. Inhabited by flies, they can be found in Chapter 2 and Chapter 3. Although harmless on contact and lacking any direct attacks, they will spit out Attack Flies and spawn multiple Black Flies and Attack Flies upon death. They behave similarly to Mulligans including audibly whimpering and evading the player. Hives have higher health than Mulligans.

 Chance to replace Hives in Flooded Caves
|
Drowned Hives behave similarly to Hives, but spit Drowned Chargers instead of flies. Upon death, they spawn 1-2 Drowned Chargers.
Nests, a new enemy in Rebirth, are a difficult variant of the Mulligan. Instead of flies, they are swollen with Spiders which they will periodically spit. Nests share the same high health as Hives. On death they can spawn Trites or Big Spiders.
Lepers are a Mulligan variant introduced in Booster Pack #5. Lepers are initially afraid of the player and will actively try to avoid them.
For every 15 damage they take, a chunk of their head, known as Leper Flesh, will detach and start erratically chasing Isaac. Lepers deal contact damage if they are in this state or below. Lepers can naturally be found with some head chunks missing.
After being reduced to 15 health, Lepers will start chasing Isaac, leaving a trail of damaging red creep. Upon death they leave behind one Leper Flesh and a slightly grayer Gusher.

 Spawned from Lepers
|
Leper Fleshes are head chunks from Lepers that erratically chase Isaac and are spawned from Lepers for every 15 damage they take. Upon death, they fire a red blood shot at Isaac.
They will die if Isaac walks into them, though this will result in Isaac taking damage.
- You can tell a Mulligan or Mulligoon is about to self detonate when they hold their breath and inflate.
- The Mulligan, Mulligoon, Hive, and Swarmer are all the same monster in stages of decay and infestation.
- In the original Binding of Isaac, when a Mulligan self-detonated, the explosion would not break rocks.
- In the game's files, there is an unused Mulliboom self-detonation animation, an unused graphic of a Hive with a Mulligoon's face, and an unused graphic of the
Drowned Hive with a blue-tinted Mulligoon's face.
Drowned Hives, along with Drowned Chargers (name of Drowned Charger wasn't revealed), were revealed for Afterbirth in this official Binding of Isaac post.
A leper is a person who suffers from leprosy, or a contagious disease that causes discoloration and lumping on the skin plus disfigurement and deformities.

Upon use, spawns a random item from the current room's item pool, with a chance to spawn A Lump of Coal or The Poop instead.

- Buddy in a Box
Grants a familiar with a random tear effect. The familiar is randomized at the start of each floor.
- Lil Delirium
Grants a familiar that transforms into another random familiar every 10 seconds.
HairpinFully recharges Isaac's active item upon entering a boss fight.
Wooden CrossGrants a Holy Mantle shield upon entering a new floor, which protects against one hit of damage.
Butter!Using an active item will drop it onto a pedestal on the ground. Taking damage may result in dropping a passive item.
Huge GrowthMakes Isaac large for the current room, increasing his range and damage.
Ancient RecallSpawns 3 random cards.
Era WalkFor the current room, enemies are slowed, Isaac's speed is increased, and his shot speed is decreased.

Grants an active item charge every 15 enemies killed.

Upon use, makes a random item from a shop, devil deal, or black market free.
- Telekinesis
Upon use, for the next two seconds, any enemy shots that come near Isaac are held in place. After the charge is depleted, they are fired back as tears.
- Moving Box
If the box is empty, picks up all items and pickups in a room and stores them. If the box is not empty, places all stored items and pickups in the current room.
- Technology Zero
Connects Isaac's tears with electricity that can damage enemies.
- Leprosy
Upon taking damage, grants a body part as an orbital, which blocks enemy shots and deals 7 damage per tick to enemies it comes in contact with. The orbitals will be destroyed if they take enough damage from enemies or enemy shots.
Filigree FeatherAngels drop items instead of key pieces when defeated.

Grants a random Harbinger familiar that changes every 10 seconds.

Summons a controllable ghost that can perform various tasks depending on what it targets.
- Angelic Prism
Grants a prism orbital with a large orbital radius. Tears that pass through the prism split into four multi-colored tears.
- Pop!
Replaces Isaac's tears with eyeballs that can bounce off of each other. The eyeballs are not limited by range, and will instead dissipate when they stop moving or when hitting an enemy or obstacle.
Door StopThe last door Isaac went through stays open, allowing him to leave the room even if enemies are in it.

Grants a reward if all enemies in a room are killed in the order they are marked.
- Haemolacria
Tears are now fired in an arc. Upon hitting the floor, an obstacle, or an enemy, they burst into many smaller tears in all directions. Increases tear delay.

Isaac's tears decelerate as they travel. Upon stopping or hitting an obstacle, they burst into 8 tears in all directions. Tears can be fired into other tears to feed them, increasing their damage.

Spawns a random pill, and grants a small Spewer familiar that charges up and fires different types of creep.
- Marbles
Spawns 3 random trinkets. Taking damage has a small chance to swallow Isaac's currently held trinket, gaining its effect permanently.

Spawns an egg familiar, which cracks and turns into a random friendly enemy when Isaac is hurt. The egg regenerates upon entering a different room.
- Flat Stone
Tears bounce off the floor. Tears that hit an enemy after bouncing deal splash damage to all nearby enemies for 25% of Isaac's tear damage.
- Marrow
Grants one bone heart and spawns three red heart pickups.
- Slipped Rib
Spawns a bone familiar that circles around Isaac and reflects enemy shots.
- Hallowed Ground
Spawns a white poop familiar, which turns into a white poop when Isaac is hurt.
- Pointy Rib
Spawns a bone familiar that can be aimed with the fire buttons and does contact damage.
- Book of the Dead
Upon use, spawns a friendly Bony or an orbital bone for each enemy killed in the current room.
- Dad's Ring
Creates a ring of light around Isaac that petrifies enemies in contact.
- Divorce Papers
Spawns a Mysterious Paper, increases tears, and grants a bone heart.
- Jaw Bone
Spawns a familiar that flies the room while firing, dealing a flat 7 damage to enemies that come in contact and retrieving pickups.
- Brittle Bones
Replaces all red hearts with 6 empty bone hearts. Upon losing a bone heart, Isaac fires bones in 8 directions and permanently gains a tears up.
- Broken Shovel
While held, causes multiple Mom's Foots to come down and stomp on Isaac. One of two parts necessary to create Mom's Shovel.
