Sheol is one of two unlockable floors following The Womb/
Utero/
Scarred Womb, the other being the
Cathedral. After defeating Mom's Heart, a devil room may spawn containing a trapdoor leading down to Sheol. Unlocking and defeating It Lives will always spawn a trapdoor leading to Sheol, as well as a Beam of Light leading to the Cathedral. Sheol can also be accessed using
We Need to Go Deeper! from The Womb II, even if Mom's Heart has not been defeated. It can also be entered if a Joker is used on the Womb II, as it may bring the player to the Devil Deal room with a trap door.
Sheol has only one floor, and the boss is always Satan.

Walks towards Isaac and attacks by launching its head towards him. Turns into a Red Maw or Gusher upon death depending on the killed body segment. Occasionally separates at will. |

Trivia[edit | edit source]
- In Hebrew, Sheol is translated as "place of the dead"; it is described as the common grave of humans in Jewish religious lore (including the Old Testament), making it the Abrahamic version of the underworld.
- Sheol is the 9th level in the game (without there being a Curse of the Labyrinth). The 14th-century novel Dante's Inferno, one of the first books to describe hell in detail, notes that the 9th layer of hell is frozen. On Isaac's last will, it says he died in "a cold place" if he dies in Sheol.
- The 9th layer of hell is said to be where Satan is imprisoned, indicating the reason for Satan's presence in the area.
- Like in all chapters, small noninteractive objects can be found on the ground. One of these decorations is a drawing of a triangle that may be a reference to the Triforce, an iconic relic from The Legend of Zelda series.

The music for Sheol is titled Duress. The term duress means intimidation or coercion and usually denotes violence.
