- +0.3 speed.
- Decreases Isaac's size, making him less likely to be hit.
- Upon pickup, gives Isaac a random pill. If he is already holding a consumable, it is forcibly dropped to the ground.

- Caffeine is a stimulant in coffee and other drinks, relevant to the speed boost. It was also rumored, but isn't proven, to stunt growth in children, as per Isaac's decreased size and emaciated appearance with it.
