So possession of the negative is not actually required to unlock ultra hard. Killing the lamb and then unlocking the negative and traveling to the dark room is good enough(ie. you killed satan(having already done a sacrifice room run prior and killed the lamb previously) for the fifth time and used a sac room it will unlock upon arrival to the dark room). Uh yeah I know the amount of bullshittery I did to test that was pretty awful(still haven't unlocked it lives! but I now have ultra hard unlocked XD) - anonymous

I have unlocked Bethany and yet the Bloody Mary challenge is not unlocked for me, so perhaps that is not the only requirement (I don't have all the starting items for that challenge, so perhaps you need all of those as well) -anonymous

"I RULE!" requirements in Repentance[]
It seems the requirements listed are incorrect.
I just beat Mega Satan for the first time, and it did not unlock.
Of note, I have beaten ??? (twice, as Isaac and Blue Baby), have yet to beat The Lamb. Also, I have not beaten Satan as Isaac (to unlock Mom's Knife).
My guess is that unlocking Mom's Knife is still a requirement. That is my next run, and I will report back if it unlocks Challenge #26.
EDIT: Can confirm, still requires you unlock Mom's Knife by beating Satan as Isaac.
Suggestion for small edit[]
Would it be alright to add "16 minutes after the challenge starts, Isaac will start taking damage every 10 seconds." to the conditions of the SPEED! challenge? I feel like that's a significant enough detail to warrant mentioning.

Crown trinkets not spawning item rooms.[]
So I think it's worth adding that the bloody, wicked and holy crown trinkets won't actually spawn item rooms on challenges that don't allow them to spawn. It's a cool detail worth mentioning.
Why are the starting items in some of the challenges not displaying correctly?[]
for example high brow. links are broken on both chrome and edge, idk how to edit table
