- Abyss: When Isaac uses Abyss to absorb The Inner Eye it creates three locusts instead of one.
- Cursed Eye: At full charge, fires 3 rows of 4 tears, resulting in 12 tears.
- At full charge, fires 3 rows of 5 tears, resulting in 15 tears.
- Doctor's Remote / Epic Fetus: Replaces the single missile strike with a barrage of three consecutive missiles.
- The Inner Eye: Effects stack, resulting in 6 tears being shot with no further decrease in fire rate.
- Adds an additional tear with no further decrease in fire rate. This can be stacked up to 16 tears.
- The Ludovico Technique: The tear gains two orbiting tears.
- Mom's Knife: 3 knives are fired.
- Polyphemus: Tear rate is not lowered further. Damage is increased by a flat +5 instead of a normal Polyphemus boost.

- Removes the fire rate penalty while reducing damage to 80%. Additional copies will add one additional tear with no further change to damage/fire rate.
- Brimstone: Fires 3 beams. Affects Azazel's short-ranged Brimstone similarly.
- The Forgotten: Triples bones thrown and swung. Significant decrease in fire rate.
- Keeper: Fires 6 tears with significantly decreased fire rate.
- Fires 4 tears with significantly decreased fire rate.
- Tainted Keeper: Only fires 5 tears in the volley, while still gaining significantly decreased fire rate.
- Kidney Stone: Increase number of tears in tear barrage; however, only fires one stone.
- Lead Pencil: The barrage will activate as normal but Isaac will sometimes fire red tears.
- Loki's Horns: Only occasionally fires one tear in the other three directions.
- Mom's Eye: Only occasionally fires one tear out of the back of Isaac's head.
- Mutant Spider: Overrides The Inner Eye.
- Shoots 7 tears instead of 3. No further decrease in fire rate.
- Shoots 5 tears instead of 3. Decreases fire rate by a little.
- Monstro's Lung: Adds 5 tears to barrage with a significant decrease in fire rate.
- Sad Bombs: Does not affect the number of tears when bombs explode.
- Technology 2: The laser is not affected by The Inner Eye.
- Varicose Veins: No effect on number of tears when taking damage.

- The Inner Eye is a reference to the concept of the 'third eye', an idea popular in Eastern mysticism dictating the existence of an invisible third eye usually located on a person's forehead which would allow them to see beyond normal reality.
