Synergies
- Book of Virtues: Creates Red Heart wisps in the middle ring. Enemies killed by the wisps' tears have a 20% chance to drop a full Red Heart.
- Car Battery: Doubles the amount Yum Heart heals.
- Jacob and Esau: Heals the character who uses it for a full red heart and the other for half a red heart.
- Keeper / Tainted Keeper: Heals one Coin Hearts; no blue fly is generated.
- Sharp Plug + The Wafer / Cancer: Allows infinite Red Heart regeneration.
- Tainted Magdalene: Heals two Red Hearts upon each usage.
- Tainted Magdalene + Sharp Plug: Allows infinite usage and infinite invincibility frames.

- This item along with any form of healing other than the starting item has no effect in Have a Heart (challenge #25)
